Roll over effect in CSS - Question

Hello, In a recent post there was a link to a tutorial whereas you have 
three images and with CSS you can do a
roll over effect typically only done with javascript.  I have a question 
regarding this can you have the image within a image, as
in you have your button within a bigger graphic ? If so how would you do 
the roll over effect because as the tutorial stated
there was three graphics so I'm guessing you'd have to create three 
graphic files to represent the normal state, over state then finally
the hit state all nested within the bigger graphic file.
