Hi Eugene,
Hurray, I see 2 problems! ;-)
1. Opening the page in IE6 under Win98SE, the huge gap is dropping the 
form to my feet - but after a refresh it is coming in the right place...
2. The html-validator reports errors

Some of them are in relation with the table of the form.

Hoping the 2 problems are related too,
