Sorry this is late in coming -- very behind on my list mail, as you can see.

 From our own Bob Easton's tests, it appears that JAWS 4 does not read 
alt text on 1 x 1 images (test case 5) but does on a 10 x 1 image (test 
case 6):

I can't say whether JAWS 4 is relevant to cater to or not.  I do know 
that JAWS is very expensive, so I would guess that many of its users are 
not fast to upgrade.

I personally try to just keep the text "skip to main content" on the 
screen for the benefit of all users, and if I must hide it, I use an 
off-left technique.  More information here:
