Generic Form Layouts - no zen's garden in this regard.

I'm looking for a generic form layouts that are css smart !

Here is my  all forms contain the known 6-7 objects ( textarea,
textbox,radio,checkbox,select,multiple select etc).
Each form object comes with a caption such as First Name, Address.
Optionaly, sometimes there is additional  text coming after the object such
as "Valid email addres is required."
You always have the "required" fields indicators.
And you may have sections such as "Personal Data", "Shipment Information"
And finally, you may have "helper pop up links" for further information such
as "?" or "Help".

Do you guys happen to use the same html lay out for your forms.
If you could be so generuous to share your generic form layouts with me, I'd
appreciate it.

I wish Zen Garden had a case for forms as well. Maybe you know alternatives
in this regard!

As an ASP/SQL programmer, I'm just to trying to ease my HTML end when I
develop web applications. But I do not have enough solid ideas as to what
the most generic CSS smart HTML structures/namings etc that I should stick
to.
