Image Replacement Quirks

What are the drawbacks of using an image as a heading in the html like 
this, without any image replacement method?

Obviously, search engines don't see any text in the hx tag... do they 
pick up on the alt attribute of the image, or possibly the title 
attribute of the hx tag or the img tag ?

How about screen readers?
