I do a sorta 3 way hybrid from what y'all have said:
1) General Attributes (body, a, blockquote, etc)
2) Large layout blocks (header, body, footer) definitions for quick overview
(helps a LOT with multiple designers working on umpteen bazillion sites)
Some major subsections for a site might go here in logical order, e.g. a
block that's always set left for nav
3) Specific styles in alphabetical class name
Seems to help get the basic structure and definitions down fast and keep the
file easy to jump through.
